The Patient Complaint Line
National Spine & Pain Center, fully encourages our patients to engage in open dialogue regarding any concerns or questions that may arise. However, we recognize that some individuals may prefer to report or convey information in a confidential or anonymous manner. In order to provide our patients an opportunity to report any concerns anonymously and without fear of retaliation, National Spine & Pain Center, provides an alternative method for reporting concerns and access to “The Patient Complaint Hotline”, a confidential/anonymous way for you to report any problems, issues, or concerns relating to any specific location or visit.
The Patient Complaint Line is available 24 hours a day, 365 days a year, and we welcome you to use it.
How to Use The Patient Complaint Line
There are two ways to use The Patient Complaint Line – via phone or completing an online form:
Phone: 1-800-561-0798
· By phone, you are greeted by a trained third party interviewer who documents in detail the situation you describe. You may disclosure your name or remain anonymous.
· A report number will be assigned and the information will be relayed to the appropriate NSPC office. Using the report number, you may call or e-mail to add more information

· Complete all screens of the web form online. You may disclose your name or remain anonymous. When you have completed the web form press the submit button.
